Requirements
- Duly accomplished Application Form or Letter of Application/Intent
- Application Fee
- Sketch map of the applied area
- If applicant is a corporation, association or partnership a true copy of its Articles of Incorporation/ Partnership and by-laws duly certified by the Securities and Exchange Commission (SEC)
- If applicant is a cooperative, a true copy of the Articles of Incorporation and by-laws duly certified by the Cooperative Development Authority (CDA)
- If applicant uses a trade name; a certificate of registration from the Department of Trade & Industry (DTI)
- Certified Copy of Latest Income Tax Return
- Proof of Capitalization
- Endorsement from the concerned Local Government Unit (LGU)
