  2. Certificate of Registration of the Peoples Organization (PO)
  3. List of Officers and Members of the PO, including their respective addresses
  4. Resolution of the governing body, designating the authorized representative to apply/sign documents for and in behalf of the organization
  5. Appropriate clearance from the National Commission on Indigenous Peoples (NCIP)
